New Patient Registration
Patients living within the practice area of Ossett, Horbury, Middlestown, Netherton and Lupset are welcome to register and our reception staff can confirm your eligibility from your postcode.
In this respect please bring along proof of address such as a recent utility bill.
You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
Medical treatment is available from the date of registration. Registration may not always be completed on the same day forms are received at reception. Please contact reception for further information.

Named GP
All patients registered with our Practice have a named GP. This does not affect you being able to see the GP of your choice.
To find out who your named GP is, please ask at reception.
